Instructions
The purpose of this request is to recognize that life happens, and sometimes our school work gets away from us. In lieu of making personal requests for an extension or late submission, please submit this form to indicate how you would like to use your (one and only) second chance. Specific rules and instructions are listed below.
1. Each student is allowed to submit one Second Chance Request for this course during the term. Only one request will be considered.
2. Answer every question included on the request. Incomplete requests will not be approved.
3. A rationale is not required or requested.
4. Late/incomplete discussion or session requests must be submitted by 11:59pm on Monday, July 4th. Approved second chance submissions must be completed by 11:59pm on Friday, July 8th.
